Residency on EAD/H1B/J1 Visa

I am currently on H4 visa and possess EAD but never worked so far (husband on H1B, EB3, PD-2006, India). I am planning to join residency on July 2011. Have two offers (1) community hospital which is ready to offer H1B visa if needed and (2) from top University hospital which will only issue J1. If I accept H1B visa will I lose my EAD & AOS (I485) status? If that is the case I will join the residency program (3 years) with EAD. Not sure if we will have EAD for 3 yrs (company layoffs). Community hospital is a non-profit organization and will not have any problem getting H1B in case EAD is lost. On the other hand university hospital is the best hospital but in case I lose EAD will have to accept J1 (J1 needs to go back to home country or obtain waive-not easy) what is the best option for me?
