Q.

H1 to F1 COS (Doc checklist,I-94 copy or original,F1 start date)

Hi,
I am planning to switch from H1B to F1 this spring (enrolling in the phd program). I was in F1 (graduated in MS) prior to moving to H1B
1. Can you provide me a checklist of documents to be sent to USCIS?
2. Should I send my original I-94 or a copy (I-539 instructions says an original is needed though)? Just want to be double sure.
3. In which section in the I-539 can I mention my start-date for F1 status (I wish to be in H1 till December 15th 2010 .School starts Jan 5th 2011)?
