Q.

H4 to H1 to H4

My spouse came to US on H4 and later got an H4 ext.approval(valid till June-2010).Then she got her H1 through a consultant and got the approval notice(valid from 10/1/2008 till 10/1/2010).She was on bench and doesnt have any paystub.She left USA only on 03/31/2009.
She would like to come to USA after 2 months on H4(based on my H1).
1.What are the chances of success in stamping in India?
2.Would the officer ask for paystub of her previous H1?
3.What should be the answer she would give to following 3 question in DS156 and DS157?
Q30:Have you ever been issued US visa when, where,what type of visa?
Q38:Have you ever violated the terms of a US visa,or been unlawfully present in,or deported from, the United States?
Q12:Not including your current employer, List your last two employers?
