H1B-B2 transfer doucments and AR11 related question

Dear Mr. Michailov,

I have already e-filed the I-539 for transfer from H1B to B2 as I was laid-off from work recently.

What are the basic list of documents I need to mail to USCIS? Will the list included below be enough
1) Passport copies
2) I-94 copies
3) I-797A copy
4) Termination letter from company
5) Bank Statements

I have given my relatives address in the I-539 as I will be moving out of my present location in a few weeks. Should I fill out a AR-11 for change in address to my relatives address?

Thanks in advance for answering my questions.
