Visa Stamping

Hi
I came to US on F1 visa in Dec '05
Completed my masters and worked from oct'07 in an EVC model.
Then in Feb'09 my client ended the contract and my employer immediately applied for h1 revocation. I got another employer to sponsor my H1 and he applied in mar'09.
This H1 got rejected in jul'09 and my employer appealed the decision.
Soon after this my old employer again got a job for me at the same client and applied for H1 which was approved in aug'09 but I didnt get I-94 and I had to go for visa stamping to my home country.
I got the visa stamping and from sep'09 I'm working with the same employer and client. In this situation
(1) In the DS-160 where it asks are you ever "out of status" should I mention yes.
(2) Can I go for stamping to canada now.
