Visa status change from H4 to H1 through a new company

I changed my Visa status in 2009 from H4 to H1 through a staffing company. I started working at client location from beginning of 2010 and got last 10 months of pay stubs. I came to India in December 2010 and went to US consulate for H1 B stamping. Consulate gave me Form 221 (G) and asked to submit more documents.
Here are my questions:
1. Cal I go back to US with an already stamped H4 Visa without submitting documents?
2. Can I go back to US in a valid H4 Visa and again change my Visa status from H4 to H1 through another company? I already got an offer from a reputed company as their permanent employee.
3. If answer to the second question is yes, then is there any problem for changing Visa status from H4 to H1 without submitting documents mentioned in Form 221(G)?
