Hi

I came to usa on H4 stamped visa on nov 2008. Got H1 on 2009 through COS stayed here which is valid till 2012. But i did not get any project till now and dont have pay stubs. Now i am planning to go to India and go for H4 stamping.

For the Question in D160 form About Previously Issued Visa do i need to mention about my H1 visa.Since i have not got stamped in my passport.
I don't have I797 Form for H4 . Do i need it for my h4 stamping.
my husband orginal h1 petition or copy is enough?
If VO ask about my paystub what to answer?
Do i need any other documents( like federal tax returns, list of employees, W2 for all persons etc)from my husband's employer
