Working for Company A, h1 approved with Company B

Hi,
I am currently working for Company A on an h1B visa. Company B has done my h1B transfer, which is approved, but I am still working for company A. I am visiting India next month.

1) Will I have any problem at the US Consultate, if I attend the visa stamping interview, using Company A's approval notice?

2) My wife is attending the visa stamping interview (using Company A's approval notice). Will this be an issue for her?

3) Can I join Company B immediately after I reach US?
