H1B LCA Question - Error in Filing

Hello,

My employer had filed LCA last week after receiving PWD from national prevailing wage help desk center, washington DC.

By mistake question G.7 which states: Agency which issued prevailing wage, was left blank and hence denied and asked to resubmit.

What should be entered for Agency which issued prevailing wage?

OFLC NPC or NPC or NPWHC or OFLC NPC washington, DC

Also we received the PWD last month so, for question G11.a should the YEAR SOURCE PUBLISHED BE 2010?

Thank you for your help

Regards
