H1B Transfer Approval with limited period and without I-94

Hi,
I am in USA on H1B and in a tough situation.
I have original H1B stamped from Employer A from Oct-01-2007 to Sep-16-2010.
I applied for H1B Transfer from Employer B and got approval notice last week. But the H1B transfer is approved with limited period for only one year from Jan-17-2009 to Dec-31-2009(even though I have original H1B status upto Sep-16-2010). I don't know what is the reason and USCIS didn't provide any reason.
Also, i have NOT got I-94 along with approval notice.

1). Can you please advice me what should I do in this situation?
2). Do I need to get I-94 immediatly for the new approved petition, though I have orig I-94 with validity upto Sep-16-2010? If so, Where should I go to get it?
3) Is there something went wrong by mistake by USCIS?

Thanks
Srini
