RE : Opt Card

My OPT started back in Oct of 2008,when I got the card to my surprise expiration date was after 3 years (Oct 2011).I was not sure whether it was an error on USCIS part or it is normal to give OPT for that long.I approached USCIS in July,2009 verification/rectification, they asked me to mail all my documents to their center.Almost two months have passed by and I did not hear anything back from them. I'm bit worried now on how USCIS will come back. This Oct 19th it will be one year my OPT started.
My Concerns:
1. Do I need to apply for opt extension with the present card or do I need to wait for my updates? What if they get back to me after Oct 19th?
2. What will be my status if USCIS does not reply before Oct 19th?
3. Can I go back to university? If yes what will be procedure for that?
