Q.

OPT/EAD card needed for 485/765/131 filing?

I graduated from university in Aug, 2003. The OPT/EAD card was issued to me so that I can work from Sep, 2003. I have tried to find the OPT/EAD card everywhere but unable to find the same. I have the I-20s which show the information etc. Will the I-20s be sufficient? My university or previous employers also do not seem have any copy of that.

Following information is on I-20s:

OPT requested on: 10th May, 2003
OPT dates on I-20 form: From Sep 1, 2003 to Aug 31, 2004
SEVIS Student Copy number: XX
Admission number: YY
Requested by: ZZ

I have following Other information:
OPT/EAD issued at: Houston office
Student ID number: XX
Graduation Date: Aug 9, 2003
Worked at university (on OPT) From Sep, 2003 to Jan, 2004
Started work on H1-B until now: Oct 29th, 2004
PD:Apr,2008
