Q.

Lost EAD

I lost my EAD and I need to file for a replacement. Is it the form I-90 that needs to be filed? Could I file by myself ot it needs to be done by a layer?
On the form what is the correct status I have to check?
I am wainting for my green card. MY priority date (EB-3) became current in November.
