I have Approved I-140 and Labor with Company A (EB2 PD May 2007). I also have Approved Labor and I-140 with Company Company B (EB2 PD Dec2007) (I have copies of these). But, I don't have copies of Company A's I-140/ Labor. I only have SRC Number(I-140) and Labor number(Starts with A-xxxxx-xxxxx). Due to Visa unavailability , I didn't file I-485 at all

Q1)I am filing FOIA form(G639) to obtain company A's I-140 and Labor copies. What need to be filled in the following columns in G639 form?

Q2) If I join company C, can I keep the company A's PD (even if they revoke I-140) Online status still shows approved.
